Getting Started Instructional Registration; List of information you will need.
This is a list of information you will need to complete the online registration.
- Full Name
- Address
- Phone Number
- Maiden Name (security purposes)
- Email Address
- Medical Information (Doctor’s name, Insurance Carrier, Policy Number, special needs, allergies)
- Emergency Contact Information (person other than yourself if possible and cellphone #)
- School and Grade (where and what grade in Fall 2007)
- Date of Birth
- Shirt Size - if playing for 1st time or if a replacement instructional jersey is neeeded.
- "Request With" information; player or coaches name.
Please make sure that all 4 steps have been completed and you receive an email receipt.
If you do not receive a confirmation, something is wrong and you will have to go back in and complete your registration.
